rsclass=&#8220;clean&#8220;]<\/p>\n<h2>Was ist die Hypophyse?<\/h2>\n<p>Die Hypophyse ist ein kleines endokrines Organ des Zentralnervensystems von der Gr\u00f6\u00dfe einer Haselnuss. Sie liegt in einer Aussparung der Sch\u00e4delbasis, die aufgrund Ihrer Form auch als \u201eT\u00fcrkensattel\u201c (Sella turcica) bezeichnet wird. Die Hypophyse reguliert die Synthese und Sekretion von Hormonen der Schilddr\u00fcse, der Nebennieren und der Gonaden. Zus\u00e4tzlich bildet sie im Hypophysenvorderlappen das Wachstumshormon und im Hypophysenhinterlappen (Neurohypophyse) die Hormone Prolaktin, Oxytocin und das Antidiuretische Hormon (ADH, Vasopressin).<\/p>\n<p>Die Hauptpathologien der Hypophyse betreffen die \u00fcberm\u00e4\u00dfige oder verminderte Sekretion eines oder mehrerer infolge einer Entz\u00fcndung oder eines invasiven Prozesses (Adenom, Zyste). Krankheitsbilder mit einer \u00fcberm\u00e4\u00dfigen Hormonbildung sind z. B. die Akromegalie, der Morbus Cushing und das Prolaktinom. Das klinische Bild wird durch das Hormon bestimmt, das fehlt oder im \u00dcberschuss vorhanden ist. Insbesondere bei Hypophysenadenomen, die h\u00e4ufig asymptomatisch sind und als Zufallsbefund (Inzidentalom) diagnostiziert werden, ist die Beurteilung einer \u00dcber- oder Unterfunktion entscheidend f\u00fcr das weitere Vorgehen. Von endokrinologischer Seite m\u00fcssen eine k\u00f6rperliche Untersuchung und eine Laboruntersuchung erfolgen. W\u00e4hrend hormoninaktive Prozesse h\u00e4ufig beobachtet werden k\u00f6nnen, ist die Therapie der Wahl bei hormonaktiven Adenomen meist eine operative Entfernung. Eine Ausnahme sind Prolaktinome, die prim\u00e4r medikament\u00f6s behandelt werden.
